🔴 HbA1c (Glycated Hemoglobin)
Most important long-term marker — reflects average glucose over 90 days

HbA1c shows your average blood glucose over the past 2-3 months. On GLP-1 medications, HbA1c typically improves. After stopping, it begins reversing. The danger is that HbA1c has a 90-day lag — by the time it shows a significant rise, the underlying glucose problem has been developing for months.

Cliff signal threshold: Rise of 0.25% or more between two readings taken 6-8 weeks apart. A rise from 5.4% to 5.7% is already significant.

🔴 Fasting Insulin

Cliff signal threshold: Rise of 15% or more above your personal baseline reading. For example, if your baseline was 92 mg/dL, a reading of 106+ mg/dL is a cliff signal.

Normal: Below 100 mg/dL | Pre-diabetes: 100-125 mg/dL | Diabetes: 126+ mg/dL

🟡 Triglycerides
Indicates insulin resistance and metabolic stress returning

Triglycerides often improve significantly on GLP-1 therapy. After cessation, they begin rising as insulin resistance returns and dietary patterns shift. Elevated triglycerides (above 150 mg/dL) in combination with other markers indicate the insulin resistance triad — a key cliff signal pattern.

Cliff signal threshold: Rise of 20% or more above your baseline, or crossing 150 mg/dL.

Normal: Below 150 mg/dL | Borderline: 150-199 mg/dL | High: 200+ mg/dL

🟡 LDL Cholesterol
Cardiovascular risk marker that often improves on GLP-1 and reverses after stopping

LDL typically improves on GLP-1 medications as a downstream effect of weight loss and metabolic improvement. After stopping, LDL may rise, particularly if weight is regaining. For people who had elevated LDL pre-medication, monitoring is important.

🟢 HDL Cholesterol
The "good" cholesterol — typically rises with GLP-1 use and may fall after stopping
